The Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PMJAY), launched on 23 September 2018, is the world's largest government-funded health insurance programme. It provides cashless and paperless hospitalisation coverage of up to ₹5 lakh per family per year for secondary and tertiary medical care at empanelled government and private hospitals across India.
As of May 2026, the scheme covers approximately 12 crore poor and vulnerable families — roughly 55 crore individuals — representing the bottom 40% of India's population. The hospital network has grown to 36,229 empanelled hospitals, including 19,483 government facilities and 16,746 private hospitals, spread across all 36 states and union territories. Over 43.52 crore Ayushman Cards have been issued to date, making this one of the most widely accessed welfare schemes in the country.

West Bengal was the only major state that had not joined PMJAY since its 2018 launch, as it operated its own Swasthya Sathi scheme. In May 2026, West Bengal officially joined the Ayushman Bharat PMJAY framework, bringing all 36 states and union territories under the national network. Launched on 29 October 2024, the Ayushman Vay Vandana Card is a separate purple-themed card that provides ₹5 lakh in free annual health coverage to every Indian citizen aged 70 years and above — regardless of income, family background, or existing insurance. Senior citizens already in PMJAY-eligible families receive this as an additional top-up over the family's existing ₹5 lakh cover. Those not in PMJAY-eligible families get a standalone ₹5 lakh individual cover. Over 3.4 crore senior citizens have already enrolled.

The updated Health Benefit Package (HBP) now covers 1,961 medical procedures across 27 specialities, including cardiology, oncology, neurosurgery, orthopaedics, nephrology, gynaecology, ophthalmology, and more.
